    There's all sorts of possible causes.  A medical person would have to begin the process of differential diagnosis to eliminate alternative diagnoses.  The doctor would probably begin with a physical exam, collecting information on medical history, and getting blood work to look for markers of infection or inflammation.   Those results would either provide an answer or lead to the next steps in gathering more diagnostic information
