Have you been struggling to take cold showers?

    When the matter relates to the a situation where something happening , that likely to be very brief.From this angle, the first sentence is fine and shows that you are struggling to take cold showers which is purely temporary.The 1st sentence, therefore, is correct.

       Your 2nd sentence too is correct.It implies your struggle to take cold showers is by no means a temporary action but has been going on from remote or recent past.

    Both are just fine.....They both have a start, a middle and a ending which is the law for writing English.....
