Because programming a group of people to act contrary to the programming of the rest of the game would be difficult to do and ultimately pointless. What tangible purpose would kids serve in GTA?
Even if you can't kill children, they could still be shot, stabbed, burned alive, blown up, ran over, mauled by animals, and other nasty stuff. That's a line Rockstar won't cross and it's a load of controversy they don't need. Their games are banned and protested against with what they have. If you put kids in, you can guarantee there will never be another entry in the series. They considered putting kids in GTA III, but quickly realized what a mistake it would've been.
Bethesda puts children in Skyrim for role-playing purposes. With DLC, you can be a parent along with spouse. GTA is about criminals, violence, and adrenaline. Children wouldn't fit into the setting.
